 Transportation
We will transport your injured employee by cab back to the work
site or to his/her home (within 5 miles) according to the physician's
and employer's instructions after evaluation for the initial injury.

- Chest X-ray

- DOT and Non DOT Drug Screening
- Post accident Drug Screening

Communication
After each initial employee injury visit to our facility, the physician
will call and inform you of the status of your employee. On all
subsequent visits the employee is given a status form that describes
his/her condition, work restrictions, follow-up recommendations,
referrals and appointments. A copy is also mailed to the employer
with all other pertinent information. The Doctor's First Report
is completed within 48 hours.

Materials Provided
You will receive CMH large identification posters with a map and
directions to each facility, along with Rolodex cards for your convenience.
Authorization forms for injured employees to bring with them to
the facility are also provided. A supervisor or responsible party
at the job site signs the authorization form, which should be presented
to the facility upon arrival.
